Half-baked state audit doesn't settle questions about Medicaid savings

Privatizing Medicaid saved the state of Iowa about $126 million during the fiscal year that ended June 30, according to a report State Auditor Mary Mosiman released on November 26.

I put low odds on that estimate holding up after a more diligent auditor takes office.

Even if it did, this review of “savings” was too narrow to reveal whether turning Medicaid over to for-profit companies was a good deal for Iowans.

Lessons of 2018: Both parties elected more women lawmakers than ever

Fourth in a series interpreting the results of Iowa’s 2018 state and federal elections.

The largest group of women ever to run for the Iowa legislature has produced the largest contingent of women lawmakers in state history.

For the first time, women will make up more than a third of Iowa House members and a majority of the lower chamber’s Democratic caucus.

The number of women serving in the Iowa Senate will exceed the previous record set in 2013 and 2014. In a major shift from the recent past, the women senators will include almost as many Republicans as Democrats.

I once worked at Goodwill of Northeast Iowa. I was paid $0.06 (6 cents) per hour minus meals. For one week of working there I was paid $3.24 – and went out and purchased an ice cream cone.

Prisoners are paid 25 to 50 cents per hour for their work and duties inside prison. Therefore, they are “worth more” and are “more valuable” than me while I was being paid 6 cents per hour when I worked at Goodwill.

For Republicans who go on about the “dignity of work”: for me, there was no dignity in “working”- in the back room sorting ţhings, while being bullied, emotionally abused, and shamed.

Iowans will likely elect record number of women lawmakers in 2018

A record number of women running for office in Iowa this year has translated into a record number of women who will appear on our state’s general election ballot. Iowa State University’s Carrie Chapman Catt Center for Women and Politics noted that 85 women (86 percent of female candidates on Iowa’s primary ballot) won their party’s nominations yesterday.

More women than ever will likely win Iowa House seats this November (current number: 28 out of 100). Female representation will almost certainly increase in the state Senate too and could exceed the previous record (ten out of 50 senators in 2013-2014). Follow me after the jump for details.

Boulton's conduct was unacceptable. His response is not credible

Three women have described in detail incidents of non-consensual touching by State Senator Nate Boulton, Brianne Pfannenstiel reported today for the Des Moines Register. Boulton did not deny the women’s accounts but said they did not match his recollection. He also asserted his alleged behavior “in social settings” was not comparable to harassment or assault in the workplace.

Boulton’s alleged conduct was unacceptable. His distinction is not credible. His political career is no longer tenable.

Iowa legislative recap: Constitutional amendments

Iowa lawmakers went home for the year on May 5. In the coming weeks, Bleeding Heartland will catch up on some of the legislature’s significant work that attracted relatively little attention.

Two proposed state constitutional amendments passed both chambers and could appear on the 2020 general election ballot, if the House and Senate approve them in the same form during either 2019 or 2020.

Three other constitutional amendments cleared one chamber in 2017–in one case unanimously–then stalled in the other chamber as lawmakers completed this two-year session. Those ideas may resurface next year. But since changes to the state constitution must be passed by two consecutively elected legislatures before landing on the general election ballot (the last step in the process), Iowa voters would not be able to ratify those proposals until November 2022 at the earliest.

Report highlights Iowa Medicaid horror stories; oversight bill languishes

Complaints to the Iowa Office of Ombudsman regarding privatized Medicaid increased by 157 percent last year, reflecting “systemic frustration” over cuts to health services for patients and unpaid bills for providers. Ombudsman Kristie Hirschman has “yet to be convinced” that the Department of Human Services is providing “adequate oversight” of the private insurance companies that control access to care and reimbursements, she wrote in a report released on April 2. The three outrageous examples she recounted resemble too many other tragic cases since Iowa shifted to a managed-care model for more than half a million Medicaid recipients.

Hirschman has assigned a full-time staffer to handle Medicaid-related complaints. The same issues come up again and again, more than a year after she, the state’s Long Term Care Ombudsman, and the advocacy group Disability Rights Iowa informed the DHS director about recurring problems with Medicaid managed-care organizations.

Although the ombudsman praised the legislature for “taking steps to correct some of the problems we and others have identified,” Senate Republican leaders haven’t brought up a Medicaid oversight bill that passed the Iowa House unanimously last month.

Senate confirms Iowa DHS director with four votes to spare

The Iowa Senate confirmed Jerry Foxhoven as director of the Iowa Department of Human Services in an unusually close vote on March 21.

Most of Governor Kim Reynolds’ appointees have won unanimous confirmation, as has typically been the case in Iowa for many years. Foxhoven’s nomination was controversial because of how privatized Medicaid has been managed, along with several tragedies involving abused children. Senate Democrats asked to defer consideration on the DHS director last month “until we can fully assess his leadership.”

Weekend thread: Statewide candidate edition

Iowa will soon have its first new secretary of agriculture since 2007. The U.S. Senate confirmed Bill Northey on February 27 as undersecretary for farm production and conservation at the U.S. Department of Agriculture. He should have been confirmed months ago; senators on the Agriculture Committee unanimously endorsed his nomination in October. But Senator Ted Cruz of Texas held the nomination over a Renewable Fuel Standard dispute that has nothing to do with Northey’s portfolio.

Once Northey resigns as Iowa secretary of agriculture, Governor Kim Reynolds will appoint his longtime deputy Mike Naig to fill that post for the rest of this year, the governor’s office announced on March 1. I enclose Naig’s official bio below. One of five Republicans who have said they will run for Northey’s job, Naig formally launched his campaign for that office on March 2. At this writing, only Craig Lang has qualified for the primary ballot. Other declared GOP candidates are Ray Gaesser, Chad Ingels, and Dan Zumbach. UPDATE: Northey posted on Twitter March 6, “I heartily endorse Mike Naig as our next Iowa Ag Secy. Mike has been a great partner as my Deputy Secy of ag for 4+ yrs. Mike is ready to lead. Let’s elect Mike in June & Nov!”

Mary Mosiman still distancing herself from oversight failure

State Auditor Mary Mosiman continues to defend her failure to ask tough questions as an ex officio member of the board that was supposed to oversee the scandal-plagued Iowa Communications Network.

Mosiman had a spotty attendance record for meetings of the Iowa Telecommunications and Technology Commission, which didn’t notice “misspending, cronyism and self-dealing” at ICN for years. Whistleblowers came forward last summer, prompting a special investigation that led to the firing of three top officials.

During a recent meeting with state lawmakers, Mosiman contended she had no reason to believe the ICN was being mismanaged and indicated she didn’t see her board role as relevant to her job as state auditor. She also revealed she hadn’t been aware that voting members were paid to serve on the technology commission, even though their salaries were written into Iowa Code.

Who's who in the Iowa Senate for 2018

The Iowa Senate begins work today with 29 Republicans, 20 Democrats, and one independent, former Republican David Johnson.

I enclose below details on the majority and minority leadership teams, along with all chairs, vice chairs, and members of standing Iowa Senate committees. Where relevant, I’ve noted changes since last year’s legislative session.

Just six senators are women (five Democrats and a Republican), down from ten women serving in the chamber in 2013 and 2014 and seven during 2015 and 2016. All current senators are white. To my knowledge, the only African-American ever to serve in the Iowa Senate was Tom Mann, elected to two terms during the 1980s. No Latino has ever served in the Iowa legislature; in 2014, Nathan Blake fell 18 votes short of becoming the first to join the Senate. No Asian-American has served in the state Senate since Swati Dandekar resigned in 2011.

Some non-political trivia: the 50 Iowa senators include two with the surname Johnson, four Marks, and two men each named Bill, Richard (Rich and Rick), Robert (a Rob and a Bob), Dan, Jim, Tim, Tom, Jeff, and Charles (one goes by Chaz).

IA-Gov: Boulton, Hubbell lead in early legislative endorsements

State Senator Nate Boulton and Fred Hubbell have locked up more support among state lawmakers than the five other Democrats running for governor combined.

Whether legislative endorsements will matter in the 2018 gubernatorial race is an open question. The overwhelming majority of state lawmakers backed Mike Blouin before the 2006 gubernatorial primary, which Chet Culver won. Last year, former Lieutenant Governor Patty Judge won the nomination for U.S. Senate, even though about 60 current and 30 former Democratic lawmakers had endorsed State Senator Rob Hogg.

Nevertheless, prominent supporters can provide a clue to activists or journalists about which primary contenders are well-positioned. Where things stand:

Weekend open thread: Short-sighted elected officials edition

Who knew that when you tell a state agency leader to save another $1.3 million somehow, he might cut some important programs and services? Not State Representative Dave Heaton, the Republican chair of the Iowa legislature’s Health and Human Services Appropriations Subcommittee.

Who knew that when you impeach a mayor using a kangaroo court proceeding, a judge might order the mayor reinstated while her appeal is pending? Not Muscatine City Council members.

I’m also interested to know what readers think about Iowa Insurance Commissioner Doug Ommen’s request to waive certain provisions of the Affordable Care Act in order to bring Wellmark Blue Cross/Blue Shield back to Iowa’s individual insurance market for 2018. Elements of the “stopgap” measure violate federal law; health care law expert Timothy Jost told the Des Moines Register’s Tony Leys that some parts of Ommen’s proposal are “extremely problematic” and not likely “doable.” Writing in the Wall Street Journal, Anna Wilde Mathews and Louise Radnofsky saw the Iowa developments as “a key test of the ability to modify the [Affordable Care Act] through executive authority.” Slate’s Jordan Weissmann agreed.

Will Kim Reynolds change DHS policy, or just directors?

Iowa Department of Human Services Director Chuck Palmer is retiring effective June 16, Governor Kim Reynolds announced today. Critics including Democratic State Senator Matt McCoy have called on Palmer to resign for months, charging that inadequate staffing at DHS facilitated more suffering and premature deaths among abused children. The department’s handling of Medicaid privatization has also drawn criticism. Despite Palmer’s promises to hold the line, the DHS agreed to pay private insurance companies more for managing Medicaid. In addition, DHS officials have downplayed numerous, ongoing reports of those companies cutting back on health care services and failing to reimburse providers promptly or adequately.

In keeping with Governor Terry Branstad’s playbook when Teresa Wahlert ended her disastrous tenure at Iowa Workforce Development, Reynolds didn’t acknowledge any problems with Palmer’s management of the DHS today. On the contrary, she and acting Lieutenant Governor Adam Gregg praised Palmer’s work in the official press release, enclosed in full below.

Branstad/Reynolds claims on Medicaid "not matching reality"

Real-world data don’t match figures Governor Terry Branstad and Lieutenant Governor Kim Reynolds released yesterday in order to demonstrated the alleged “continued success of modernizing our state’s Medicaid program.”

April 1 marked a year since Iowa allowed three private insurance companies to manage care for more than half a million Medicaid recipients. The shift was disruptive for thousands of Iowans as well as for caseworkers and service providers, some of which went out of business. Reimbursement problems and cutbacks to care are still affecting many people, Chelsea Keenan reported in this retrospective on the first year of the policy.

Although privatization was supposedly designed to save money and bring predictability to the state budget, the Branstad administration agreed last fall and again in February to hand over millions more state dollars to the managed-care organizations (MCOs), unlocking some $225 million in extra federal funding for the corporations, which have much higher administrative costs than Iowa’s state-run Medicaid program did.

I enclose below the latest deceptive official statements about the “modernization,” along with a demolition fact-checking job by Democratic State Senator Liz Mathis. I’ve also included independent State Senator David Johnson’s reaction to what he called a “lousy, lousy” press release. While still a member of the Republican caucus during the 2016 legislative session, Johnson worked with Democrats trying to halt Medicaid privatization or at least provide stronger legislative oversight of the program.

The Iowa Hospital Association can’t substantiate the Branstad/Reynolds claims on hospitalization rates, Tony Leys reported yesterday for the Des Moines Register. Excerpts from that story are at the end of this post.

On a related note: thousands of Iowans who follow this issue closely are mourning Rhonda Shouse, who died unexpectedly in late March. I never met Rhonda in person, but we communicated through social media, and I admired her relentless advocacy on behalf of those adversely affected by Medicaid privatization. Keenan marked her passing in the Cedar Rapids Gazette, and Leys did so in the Des Moines Register. May her memory always be for a blessing.

Iowa Democrats, talk less about ALEC and more about people's lives

Thousands of Iowans will suffer brutal consequences from the two major bills Republican senators approved Monday. House File 295 blocks local governments from raising the minimum wage. Once Governor Terry Branstad signs the bill, thousands of people working in Linn, Johnson, and Wapello counties will get an immediate pay cut. Some 25,000 people in Polk County will be stuck earning $7.25 an hour, instead of getting a raise to $8.75, beginning next week. House File 518 will make it harder for employees to file workers’ compensation claims and will vastly reduce benefits for those who do qualify, especially anyone with a shoulder injury.

Both bills passed on party-line 29-21 votes after Republicans had rejected every effort to mitigate the harm done to working people.

As each Democratic amendment went down during hours of debate on the Senate floor, feelings of sadness, disgust and anger came through in the speeches of some Democrats and independent State Senator David Johnson. Why are you doing this, several asked their GOP colleagues. You don’t have to follow your floor manager, some pleaded. You can reject the “shameful” attempt to target poor people or those affected by life-altering workplace accidents.

Another dismal day in the Iowa legislature provoked an outpouring on social media, where progressive activists have mobilized this year in response to the Republican agenda. A measurable wave of “greater grassroots activism on the political left” is one of the few bright spots in the national landscape. In Iowa too, ordinary people are contacting their state lawmakers in record numbers and showing up to challenge them at district forums.

Watching these discussions unfold, I’ve noticed a reflexive tendency to blame one destructive Iowa GOP bill after another on the Koch brothers or the American Legislative Exchange Council. The more Democrats make the conversation about Koch money or ALEC, the easier it is for Republicans to avoid talking about the real-world consequences of their actions.

Everything you want to know about Iowa's horrible new collective bargaining law

Republicans in the Iowa House and Senate voted today to dramatically reduce collective bargaining rights for some 180,000 public employees, following approximately 27 hours of debate in the Iowa Senate and fourteen and a half hours of debate in the Iowa House. GOP leaders moved House File 291 and Senate File 213 simultaneously through both chambers in order to speed up the process.

Democrats had offered dozens of amendments to the bills, which were published for the first time on February 7. Instead of allowing full discussion of every amendment, GOP leaders moved to cut off debate at a “time certain” today. That maneuver had never been used in the Iowa Senate and has been invoked only rarely in the Iowa House–including to end debate on the collective bargaining bill Republicans passed in March 2011. Debate ended in the Iowa House at noon, after which the majority quickly voted down all the remaining amendments with no discussion. Six Republicans joined all 41 Democrats to vote against the bill on final passage. Two of them, Tom Moore and Dave Heaton, are former teachers. Clel Baudler is a retired state trooper. Andy McKean and Shannon Lundgren were just elected from eastern Iowa swing districts, where registered Democrats outnumber Republicans. McKean is also very familiar with Chapter 20 as a former county supervisor and longtime state lawmaker. I don’t know why Mary Ann Hanusa opposed the bill. UPDATE: Hanusa did not respond to my request for comment, but I learned from another source that she is also a former teacher who works in education administration.

Senators debated all night long Wednesday into Thursday morning, with Republicans voting down every Democratic amendment. Independent State Senator David Johnson voted with Democrats on all the amendments and joined them in giving several passionate speeches. Few Republicans in either chamber chose to speak in favor of the bills, aside from Senate Labor Committee Chair Jason Schultz, House Labor Committee Chair Dave Deyoe, and State Representative Steven Holt, who floor-managed the bill and distinguished himself as the legislature’s least convincing liar. The Des Moines Register’s William Petroski summarized some of the important Democratic amendments. I didn’t stay up to watch the whole debate, so would welcome examples of some of the most absurd Republican comments, like State Senator Mark Chelgren accusing Democrats of “stalling” while his party had shown an “incredible amount of patience.” Nothing says “patient” like making sweeping changes to a 43-year-old law, affecting 180,000 Iowans, after only nine days in the legislature.

Senate leaders ended debate at 2 pm Thursday, after which Republicans voted down the remaining Democratic amendments, then substituted the text of the House bill for the Senate bill, to get the legislation to Governor Terry Branstad more quickly. Branstad’s chief of staff, Michael Bousselot, spent the final hours of debate in the Senate chamber. House File 291 eventually passed on a 29-21 Senate vote.

Iowa’s largest public-sector union, AFSCME Iowa Council 61, plans to file a lawsuit claiming the new law is unconstitutional, presumably because of the way it grants more bargaining rights to “public safety” workers than to others, many of whom do dangerous jobs. I enclose below statements about the bill by legislative leaders from both parties, as well as documents prepared by Iowa House Democratic and Republican staff, which discuss in more detail how House File 291 will affect collective bargaining rights for different types of public employees. Regarding substantive impacts, I also recommend the recent guest posts here by state employee Ruth Thompson, University of Northern Iowa Professor Chris Martin, and attorney James Larew, who predicted that today’s action “will be remembered as the most destructive blow to our ability to govern ourselves fairly and efficiently in nearly half a century.”

GOP spin notwithstanding, collective bargaining “reform” in Iowa was designed primarily with political goals in mind, like similar measures in other states. Republicans know that crippling public sector unions will make it harder for Democrats to win elections.

Although Republicans repeatedly claimed during the House and Senate debates that their bill would help local governments, Chapter 20 has worked so well that more than 140 school districts rushed to sign new contracts with the teachers union before the legislature acted. Boards of supervisors in several large counties passed resolutions condemning the proposal. Linn County Supervisor Brent Oleson’s case against the bill is convincing.

Controversies over education spending and Planned Parenthood funding brought out many activists earlier in the legislative session. This weekend, the overwhelming majority of attendees wanted to talk about the Republican bill to eviscerate Chapter 20, Iowa’s law that has governed collective bargaining for public employees since 1974.

IA-Gov: Democrat Jon Neiderbach launching campaign soon

Jon Neiderbach, the 2014 Democratic candidate for state auditor, plans to announce a campaign for governor by the end of the week, he confirmed to Bleeding Heartland. A New York native who has lived in Iowa since attending Grinnell College during the 1970s, Neiderbach has in-depth knowledge of state government, having worked for the Legislative Fiscal Bureau and later in the Iowa Department of Human Services. He also served a term on the Des Moines School Board and has been an attorney in private practice since 2012. He campaigned for a few months in Iowa House district 43 during the last election cycle but withdrew from that race before the filing period.

Last year, Neiderbach was an active supporter of Bernie Sanders for president, and he will need support from that part of the Democratic base in a primary likely to include better-funded candidates. His new Twitter handle is @Neiderbach4Gov, and he’s on Facebook here. A campaign website will launch soon. I enclose below a short bio released during Neiderbach’s 2014 race for state auditor.

The field of Democratic challengers to future Governor Kim Reynolds will expand further. Rich Leopold is already campaigning around the state, and former Iowa Democratic Party chair Andy McGuire is expected to make her candidacy official within a month or two. (However, when I asked McGuire at the recent State Central Committee meeting when she was going to announce, the mother of seven grown children answered with a joke: “Oh, do I look pregnant?”)

The rumor mill sees State Representative Todd Prichard as a likely gubernatorial candidate too.

State Senator Liz Mathis told a Democratic gathering in Des Moines this month that she thought about running for governor in 2018, “but I don’t believe it’s in the cards for me.” I’ve talked to several Democrats who hope she will reconsider, including AFSCME President Danny Homan. (He is not in McGuire’s fan club.) Terry Branstad ruled out running for governor in May 2009 but changed his mind a few months later.

Rich Leopold becomes first Iowa Democratic candidate for governor

Vowing to be an outsider who can bring a “different kind of government” to Iowa, Rich Leopold just announced in a Facebook live appearance that he will run for governor as a Democrat in 2018. I enclose below his news release and a statement of “four cornerstones” that will guide his candidacy, along with a transcript of his comments on video. Leopold’s campaign website is here and his Facebook page is here.

A first-time candidate for office, Leopold stands apart from the “lobbyists, special interests, and the insider’s club that for far too long has run our government” and “is free from the generations of deal-making and permanent campaigning that has poisoned the capitol,” his “cornerstones” document declares.

Leopold has government experience at the local, state, and federal level. He served as Iowa Department of Natural Resources director during Chet Culver’s administration from 2007 to 2010, when he took a job with the Midwest Region of the U.S. Fish and Wildlife Service. He later worked for the Dickinson County Conservation Board and since 2013 has been with the Polk County Conservation Board, where he is now director. (Disclosure: I joined the board of directors of the Iowa Environmental Council when Leopold was that non-profit’s executive director, shortly before he left to lead the DNR.)

Leopold also chairs the new Grow Iowa PAC, which raised about $10,000 last year and donated to eighteen Democratic candidates or committees.

No other Democrats have confirmed plans to run for governor, but outgoing Iowa Democratic Party chair Andy McGuire is widely expected to announce her candidacy early this year. If either wins the June 2018 primary, Leopold or McGuire would be the first Iowa nominee for governor since Roxanne Conlin in 1982 not to have held elected office.

Many politics-watchers expect at least one member of the Iowa House or Senate to seek the nomination as well, perhaps State Senator Liz Mathis or State Representative Todd Prichard.

UPDATE: State Senator Chaz Allen is also rumored to be considering the gubernatorial race. He or Prichard would have to give up their seats in the legislature in order to run for governor. Mathis was just re-elected to a four-year term, so could run for governor without leaving the Iowa Senate.

Democratic State Representative Jim Lykam appears to be on track to succeed the late Senator Joe Seng, despite the governor’s efforts to engineer even lower turnout than for a typical election to fill an Iowa legislative vacancy.

Just like the Phoenix who rose from the ashes, we have an opportunity to re-build, but we need to take bold steps forward to do so.

As Democrats we basically have a mutually shared goal. I believe that goal is to strengthen the Party so we are able to get Democrats elected who support our visions, values, and beliefs. Rob Hogg, Amanda Ragan to lead Iowa Senate Democrats

Iowa Senate Democrats unanimously chose Rob Hogg today to serve as Senate minority leader during the upcoming legislative session. Amanda Ragan will be the new minority whip, and the incoming assistant leaders will be Bill Dotzler, Liz Mathis, Rita Hart, Joe Bolkcom, Matt McCoy, and Herman Quirmbach.

Six Democratic senators lost their re-election bids this month, including Mike Gronstal, who had served as either minority or majority leader of the caucus since 1997. Pam Jochum, who was Senate president for the past four years, will not be on the new leadership team. Ragan, Dotzler, and McCoy were among last year’s assistant majority leaders, while Bolkcom served as majority whip.

Erin Murphy reported for the Quad-City Times,

Hogg said Senate Democrats will speak on behalf of Iowans “who need state government to work” and attempt to prevent Republicans from implementing policies that could damage the state’s economy or adversely affect its residents.

“I’m hopeful we can stop Republicans from going down a knee-jerk, partisan pathway,” Hogg said.

I see no realistic chance to stop Republicans from using their large majorities in both chambers to head down that partisan pathway. Among their likely top priorities: cutting taxes so that most of the benefits go to corporations and higher-income individuals, gutting Iowa’s 42-year-old collective bargaining law, restricting abortion rights, ending state funds for Planned Parenthood’s non-abortion services, adopting the gun lobby’s wish list (“stand your ground,” “constitutional carry,” and/or open carry), and making it harder for Iowans to vote. Republicans will almost certainly need to reduce funding for education and a variety of social net programs, such as Medicaid and child care assistance, to pay for those tax cuts.

All Democrats can accomplish these next two years is to warn ahead of time how such policies will hurt the majority of Iowans, and to “document the atrocities” after Governor Terry Branstad signs the various harmful bills into law.

After campaigning on lies, Iowa Senate Republicans still won't lay out agenda

All but one of the 29 Republicans who will serve in the Iowa Senate for the next two years elected their leaders on Friday, changing little from the group that led the minority caucus during the last legislative session.

Bill Dix moves from Senate minority leader to majority leader.

Jack Whitver moves from minority whip to Senate president.

Jerry Behn, who preceded Dix as minority leader but hasn’t been in leadership since 2012, will become Senate president pro tem. He’s the longest-serving current Republican senator.

My own state senator, Charles Schneider, moves from an assistant minority leader position to majority whip.

Dan Zumbach and Randy Feenstra will be assistant majority leaders, having been assistant minority leaders during the last legislative session. The other two assistant majority leaders are new to leadership: Michael Breitbach and Amy Sinclair, the only woman in the 29-member incoming GOP caucus.

After the caucus meeting, Dix spoke only in general terms about the new majority’s plans.

Fewer women will serve in the new Iowa Senate and House (updated)

The non-partisan organization 50/50 in 2020 has set a goal of electing 25 women to the Iowa Senate and 50 women to the Iowa House by 2020. Yesterday’s elections will bring a lot of new voices to the state capital. However, chambers that were already less diverse than most other state legislatures will become even less representative of the state’s population.

LATE UPDATE: The new Iowa House will in fact have one more female member than the chamber did in 2015 and 2016, following Monica Kurth’s victory in the special election to represent House district 89.

Republicans outspending Democrats in most Iowa Senate battlegrounds

Iowa House and Senate candidates were required to file their last pre-election campaign finance reports on Friday. In stark contrast to four years ago, Republicans are outspending Democrats in most of the contested state Senate districts. (I’ll address spending in the key Iowa House races in a different post.)

Currently, there are 25 Senate Democrats, 23 Republicans, and one independent. If former GOP Senator David Johnson makes good on his promise to remain an independent in 2017, and Democrats win the December special election to replace the late Senator Joe Seng, Republicans would need to pick up three seats to gain control of the upper chamber for the first time since 2004.

I enclose below in-kind contribution figures for the Senate districts expected to be in play next Tuesday. Candidates running elsewhere did not report large in-kind contributions from their respective parties.

Branstad will transfer more money to for-profit Medicaid managed care providers

During his long tenure, Governor Terry Branstad has typically been attentive to the concerns of for-profit corporations. So when the insurance companies picked to manage Medicaid for more than half a million Iowans reported in August that they were losing money on the deal, they got a favorable hearing.

A senior analyst from Iowa’s non-partisan Legislative Services Agency confirmed on Friday that the Branstad administration has agreed to pay an extra $33.2 million to the three Medicaid managed care providers for the contract period beginning on April 1 and continuing through June 2017. I enclose that e-mail below, along with reaction from several Democratic state senators.

Hardly a week goes by without my hearing yet another story about an Iowan hurt by Medicaid privatization. Thousands of people have been forced to change doctors or have lost access to therapy for loved ones. This summer, a survey of care providers for Iowans on Medicaid found that:

90% say their administrative costs have increased;
80% say more health care claims are being denied;
79% of providers say they are not getting paid on time;
66% say they are being reimbursed at rates lower than their contracted rates;
61% say the quality of services they can provide has been reduced;
46% have or plan to cut their services; and
28% have taken out loans while waiting to be paid.

Branstad refuses to adjust his Medicaid policy to help patients who are suffering or care providers struggling to keep the doors open. On the contrary, he keeps spinning tales (backed up by no evidence) about “protecting the interests of the taxpayers” and “stopping significant fraud and abuse.” His administration hasn’t followed up on supposed new benefits for patients and puts out impossible-to-verify accounts of “success stories.”

Critics warned that any money saved by privatizing Medicaid would come at the expense of health care for vulnerable patients. They warned that those apparent savings would evaporate once for-profit insurers demanded more money from the state, as had happened in Florida following that state’s rapid privatization of Medicaid.

Iowa enjoyed low administrative costs under the old Medicaid system. Thanks to our governor and his Republican enablers in the state legislature, taxpayers will now experience the “nightmare” scenario that has already unfolded for Medicaid patients and care providers. Connecticut saved money by moving away from managed care, back to the state-run, fee-for-service model.

Don’t expect anything that sensible from Branstad, who happily spends other people’s money when corporations come calling.

UPDATE: Branstad claimed at an October 31 press conference that the state budget will still save money thanks to efficiencies in the new Medicaid system. If those savings materialize (they didn’t in Connecticut), it will happen through companies denying health care services to Iowans.

Rob Hogg for Iowa attorney general in 2018 (updated)

Before digging into posts about the Iowa elections that just happened, I want note for the record that State Senator Rob Hogg would be an excellent candidate for Iowa attorney general in 2018. Though incumbent Tom Miller has not disclosed his plans, he has reportedly told many Iowa Democrats privately that he does not plan to seek a tenth (!) term. An open race for attorney general would likely bring out several Democratic candidates, possibly including Solicitor General Jeffrey Thompson and First Assistant Attorney General Kevin McCarthy, a former Iowa House majority leader. Hogg has a strong legal and public policy background, having clerked for two federal judges, practiced law in Cedar Rapids since 2000, and served in the Iowa legislature since 2003. He can speak knowledgeably about a wide range of issues, and he now has experience running a statewide campaign.

Hogg was gracious in conceding the U.S. Senate primary race to former Lieutenant Governor Patty Judge this evening. He lost by about 8 percent of the vote against a candidate who started with much higher name recognition and was able to run television commercials during the final two weeks of the race. He was only half a percentage point behind the front-runner in Polk County.

Hogg will be up for re-election in Iowa Senate district 33 in 2018, so unlike this year, he would have to give up his seat in the legislature in order to seek higher office. I hope he will consider it. I enclose below the official bio from his Senate campaign website.

UPDATE: Several readers have suggested Hogg should run for governor in 2018. He would be a strong contender for the position too. I assume a number of Democratic state senators (Janet Petersen, Liz Mathis, Pam Jochum) will be looking at that race as well.

SECOND UPDATE: Multiple sources have contacted me to say Miller shows all signs of running for re-election in 2018. So either he has changed his mind, or sources who heard last year he was planning to retire were misinformed. In that case, many Democrats will be urging Hogg to run for governor in 2018.

Never let it be said that the 2016 Iowa legislature accomplished nothing

In four months of work this year, Iowa lawmakers made no progress on improving water quality or expanding conservation programs, funded K-12 schools and higher education below levels needed to keep up with inflation, failed to increase the minimum wage or address wage theft, let most criminal justice reform proposals die in committee, didn’t approve adequate oversight for the newly-privatized Medicaid program, opted against making medical cannabis more available to sick and suffering Iowans, and left unaddressed several other issues that affect thousands of constituents.

But let the record reflect that bipartisan majorities in the Iowa House and Senate acted decisively to solve a non-existent problem. At a bill-signing ceremony yesterday, Governor Terry Branstad and supporters celebrated preventing something that probably never would have happened.

Iowa Democrats trying to add autism insurance coverage to budget bill

Earlier this month, Claire Celsi informed Bleeding Heartland readers about the demise of an autism insurance bill after State Representative Peter Cownie refused to bring the measure up for a vote in the Iowa House Commerce Committee, which he chairs.

As both Autism Awareness Month and the 2016 legislative session wind down, Democrats in the Iowa House and Senate have been working to add the same requirements to a must-pass budget bill. Follow me after the jump for background and where things stand in this fight.

Prospects for increasing diversity in the Iowa legislature (post-filing edition)

Now that the deadline to compete in the Democratic or Republican primaries has passed, the field of candidates is set in most of the 100 Iowa House districts and 25 Iowa Senate districts that will be on the ballot this fall.

It’s time for a first look at chances to increase diversity in the state legislature for the next two years. The proportion of white lawmakers is unlikely to change, while the proportion of women could move in either direction.
